Case Manager (RN) – Bronson Battle Creek
Part-Time | 48 Hours per Pay Period | Benefit Eligible

Position Overview
The Case Manager is responsible for coordinating patient care from admission through discharge to ensure a safe, seamless transition across the continuum of care. This role focuses on assessing patient needs, developing care plans, coordinating services, and collaborating with the interdisciplinary team to achieve high-quality, cost-effective outcomes.
The Case Manager serves as a clinical resource to patients, families, and staff, ensuring appropriate utilization of hospital and community resources while supporting positive patient experiences and outcomes.
Key Responsibilities
Care Coordination & Discharge Planning
- Assess patient needs early in hospitalization and identify risks related to post-discharge care
- Develop, implement, and monitor individualized transition-of-care plans
- Coordinate discharge planning and community resource referrals
- Ensure smooth transitions between levels of care
Patient Monitoring & Advocacy
- Conduct daily rounds on assigned patients
- Monitor patient progress and document care coordination activities
- Identify barriers to discharge and intervene appropriately
- Advocate for patient needs throughout the care continuum
Collaboration & Communication
- Work closely with physicians, nursing staff, and interdisciplinary teams
- Lead and participate in multidisciplinary rounds
- Serve as a liaison between patients, families, providers, and community services
Quality & Utilization Management
- Monitor length of stay, readmission risks, and patient outcomes
- Support appropriate utilization of hospital and post-acute resources
- Track and report case management outcomes and variances
Qualifications
Education
- Bachelor of Science in Nursing (BSN) required
- Master’s degree preferred
Licensure & Experience
- Current Michigan Registered Nurse license in good standing
- Minimum of 3 years of acute care hospital experience required
- Case Management certification preferred
Skills & Competencies
- Strong clinical assessment and care planning skills
- Excellent communication and collaboration abilities
- Critical thinking and problem-solving expertise
- Ability to manage multiple priorities in a fast-paced environment
- Proficiency with electronic health record systems and documentation tools
Work Environment
This role involves a combination of patient rounding, care coordination, and documentation. The position requires frequent communication with healthcare teams and patients, as well as prolonged computer use and typical physical demands associated with hospital settings.
