Hospice Nurse Case Manager -242750 New

We are seeking a compassionate and skilled Hospice RN Case Manager to join our team. This role offers the flexibility to start your day from home and provide high-quality, patient-centered care in the field. Caseload is approximately 10 patients, with 3–4 visits per day.

Key Responsibilities:

• Admit and assess patients under physician orders to determine hospice eligibility
• Develop and revise individualized care plans addressing patient-specific needs, goals, and medical conditions
• Perform thorough physical, emotional, and psychosocial assessments at regular intervals
• Monitor and document changes in patient condition, and communicate updates to the interdisciplinary team and physicians
• Administer medications and treatments as prescribed, including wound care and symptom management
• Coordinate care with other disciplines such as social work, chaplain services, and home health aides
• Provide patient and family education on disease progression, medication administration, symptom management, and end-of-life care
• Ensure compliance with all state, federal, and hospice-specific regulations and standards
• Participate in interdisciplinary team (IDG) meetings and contribute to collaborative care planning
• Complete timely documentation of visits, care updates, change orders, and recertification paperwork in the EMR system (Point Care)
• Develop and execute discharge plans beginning at admission, including completion of all required documentation
• Support and guide patients and families through end-of-life decision-making and bereavement preparation
• Maintain timely communication and coordination with clinical leadership to ensure seamless patient care
• Adhere to agency policies, including timely submission of visit notes, supervisory notes, and care plan updates

Qualifications:

• Active RN license in the state of Michigan
• Hospice experience preferred; new grads with Point Care experience will be considered
• Proficiency in Point Care (EMR)
• Valid driver’s license, reliable vehicle, and proof of auto insurance
• BLS certification

Location: Kalamazoo/Battle Creek, MI

Schedule: Monday–Friday, 8:00 AM – 5:00 PM

Mileage Reimbursement: $0.67 per mile
