HYBRID Case Manager Registered Nurse (RN) – Occupational Health New

Territory: Kalamazoo & Battle Creek, MI Area

Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM

Employment Type: Contract-to-Hire

Position Overview:

We are seeking a Hospice RN Case Manager to provide high-quality, patient-centered care to individuals in the Kalamazoo and Battle Creek areas. This is a full-time, weekday-only position that allows you to start and end your day from home, offering flexibility and autonomy in your schedule.

Key Responsibilities:

• Manage a caseload of approximately 10 patients, averaging 3–4 visits per day
• Travel to patient homes to assess and deliver hospice care
• Perform wound care, medication assessments, and symptom management
• Collaborate with interdisciplinary team members to ensure comprehensive care plans
• Provide emotional and clinical support to patients and their families
• Visit the local office only as needed (e.g., to pick up supplies)

Compensation & Benefits:

• Mileage Reimbursement: $0.67 per mile
• Cell Phone Stipend Provided

Qualifications:

• Hospice experience preferred (New grads considered if they have PointCare EMR experience)
• Active RN license in the state of Michigan
• Experience with PointCare or similar EMR systems
• Reliable personal vehicle and valid driver’s license
• Proof of car insurance
